Dallas Ebola patient was sent home as a result of a flaw in software used by many hospitals

Before Thomas Eric Duncan was placed in isolation for Ebola at Dallas’ Texas Health Presbyterian Hospitalon 28 September, he sought care for fever and abdominal pain three days earlier, but was sent home. During his initial visit to the hospital, Duncan told a nurse that he had recently traveled to West Africa — a sign that should have led hospital staff to test Duncan for Ebola. Instead, Duncan’s travel record was not shared with doctors who examined him later that day. This was the result of a flaw in the way the physician and nursing portions of our electronic health records (EHR).EHR software, used by many hospitals, contains separate workflows for doctors and nurses.... However, we have identified a flaw in the way the physician and nursing portions of our electronic health records (EHR) interacted in this specific case,” the hospitalwrote in a statement explaining how it managed to release Duncan following his initial visit.
According to NextGov, EHR software used by many hospitals contains separate workflows for doctors and nurses. Patients’ travel history is visible to nurses, but such information “would not automatically appear in the physician’s standard workflow.” As a result, a doctor treating Duncan would have no reason to suspect Duncan’s illness was related to Ebola.
Roughly 50 percent of U.S. physicians now use EHRs since the Department of Health and Human Services (HHS) began offering incentives for the adoption of digital records. In 2012, former HHS chief Kathleen Sebeliussaid EHRs “will lead to more coordination of patient care, reduced medical errors, elimination of duplicate screenings and tests and greater patient engagement in their own care.” Many healthcare security professionals, however, have pointed out that some EHR systems contain loopholes and security gaps that prevent data sharing among healthcare workers.

Proud, of course, but furious.
He had not told his mother about the incident in Afghanistan when his armoured vehicle hit an explosive, catapulting the gunner from the turret and injuring the driver.
Cpl Bainbridge was singled out for his bravery after dragging the driver to a crater and lying on top of him, shielding him from enemy fire.
However, despite his bravery, Cpl Bainbridge’s mother was not impressed. The last soldier to be awarded the MC in his regiment, the Royal Dragoon Guards, had received the honour posthumously.
“When I first told my mum about the MC she was angry,” said the soldier, now 26. “She kept saying, 'I like it, but will you stop doing dangerous things now?’ ” His answer, it seems, was a determined 'no’.
Today Cpl Bainbridge, who had to return from Afghanistan after being wounded by a Taliban grenade, will fly to Argentina to begin a 2,000-mile voyage through the Southern Ocean, regarded as some of the world’s most treacherous waters.
Accompanied by L/Cpl Keith Harbridge, who was also wounded in Afghanistan, and the explorers Justin Packshaw and David Hempleman-Adams, he will retrace part of the route of Ernest Shackleton’s Endurance expedition, which set off from Britain a century ago aiming to make the first successful crossing of Antarctica....

The incident occurred at the Parchin military compound, not far from the Iranian capital, according to the Iranian Students News Agency.
The BBC said on Monday that a pro-opposition website was also reporting an explosion at the site.
The BBC cited the pro-reform website Sahamnews as saying that the explosion on Sunday evening was “so intense that windows of buildings 15 km (nine miles) away were shattered.”
“The glare from the blast could also be seen from a great distance,” the report said....

To be eligible for 9/11 Victims Compensation Fund benefits, you must prove that you were present during or immediately after the terrorist attack. If you are applying for loss of your loved one, you must prove that she or he was present and died as a result of 9/11-related injuries or disease.
Proof of presence is not always easy to collect. More than a decade has gone by since the attacks. Paperwork and computers were destroyed in the World Trade Center collapse. Businesses have closed and owners sold their property, possibly shredding documents in the process. In the chaotic aftermath of the attack, volunteers jumped in to help without thinking about the need for proof. Possibly, you were a tourist or a visitor to the area, rather than a long-term resident or employee.
The most straightforward way to satisfy the VCF presence requirement is to obtain affidavits from witnesses who can verify you were present in the area in the days, weeks or months following the attack. Affidavits can come from a wide number of sources — if a landlord, neighbor, coworker, boss or other relevant person can corroborate your actual presence, talk to them about making an affidavit. Two affidavits are ideal, but one with a supporting document is also acceptable.
Some documents that show you or your loved one worked at the World Trade Center or the Pentagon or the surrounding areas during the relevant time include the following:

Official personnel roster

; The British Islamist firebrand Anjem Choudary has been released from police custody after he was arrested for allegedly being a member of a banned terrorist group.
Choudary and nine other radical Muslims were detained during dawn raids in London on September 25 as part of an ongoing Metropolitan Police investigation into Islamist-related terrorism.
Choudary—one of the most high-profile jihadists in the United Kingdom, and well known for his relentless resolve to implement Islamic Sharia law there—is a former spokesman of the Muslim extremist group, al-Muhajiroun (Arabic: The Emigrants).
Al-Muhajiroun—which repeatedly celebrated the terrorist attacks on the United States in September 2001—wasbanned under the UK Terrorism Act 2000, in January 2010.
Since then, al-Muhajiroun has repeatedly reinvented itself under an array of successor aliases. These include, among others: Islam4UK, Call to Submission, Islamic Path, Islamic Dawa Association, London School of Sharia, Muslims Against Crusades and Need4Khalifah, all of which have also been proscribed.
Al-Muhajiroun—founded by Choudary and the exiled preacher, Omar Bakri Mohammed—is said to have also played a major role in radicalizing Michael Adebolajo, who was found guilty of murdering (and attempting to decapitate) the British soldier Lee Rigby outside London's Woolwich Barracks in May 2013.
Choudary said Rigby would "burn in hellfire" as a non-Muslim, and also praised Adebolajo as a "martyr." He said:
"Allah said very clearly in the Koran 'Don't feel sorry for the non-Muslims.'"So as an adult non-Muslim, whether he is part of the Army or not part of the Army, if he dies in a state of disbelief then he is going to go to the hellfire."That's what I believe so I'm not going to feel sorry for non-Muslims....
